South Carolina Health Insurance Alternatives to the ACA: 2026 Guide

If you've priced HealthCare.gov plans and walked away, here are the alternatives that exist in South Carolina — ranked from most coverage to least.

1. Private shared-network PPO plans

Major-medical coverage on the Cigna, Aetna and PHCS PPO networks, medically underwritten, priced on age and health. The closest thing to an employer PPO you can buy on your own. Best for healthy people above the subsidy line. Enroll any month.

2. Small-group plans

If you own a business with at least one other W-2 employee (a spouse on payroll can count in many cases), a two-life group plan opens a different, guaranteed-issue market with employer-style networks. Often overlooked by solo owners who could add a partner or employee.

3. COBRA or continuation

If you just left a job, you can usually keep the employer plan for up to 18 months at full cost. Expensive but guaranteed, and a bridge while you compare.

4. Short-term medical

Federal rules now limit these to a few months and they exclude pre-existing conditions. Fine as a true gap filler; not a year-round answer. South Carolina didn't expand Medicaid, and most marketplace plans here are narrow-network HMOs — which is why a national PPO is the thing self-employed buyers ask about first.

What's not an alternative

Health-sharing ministries, fixed-indemnity "hospital plans," and discount cards are not insurance. They can be sold alongside real coverage, never instead of it.
